About Transitions Senior Care, LLC. Transitions Senior Care, LLC is a Colorado-based senior living placement and referral company. We help individuals and families navigate senior living options — including independent living, assisted living, and memory care — at no cost to the family. Our fees are paid directly by the senior living communities we refer to, only when a placement is made.

Information Sharing. To connect you with appropriate senior living options, we will collect personal information including names, care needs, budget, and location. This information will be shared with senior living communities that may be a fit for your situation. By completing this form, you consent to our sharing your information with those communities for the purposes of evaluating placement options. We do not sell your information to third parties for marketing purposes unrelated to senior care placement.

Contact Consent. You consent to being contacted by Transitions Senior Care, LLC and its advisors by phone, email, and/or text message regarding senior living options. You understand that advisors or referral partners assisting in your search may be independent contractors or third-party subcontractors working on behalf of Transitions Senior Care, LLC, and that any such contact is made in connection with your request for senior living guidance.

No Guarantees. While we work diligently to identify communities that match your needs, Transitions Senior Care, LLC does not guarantee placement, availability, or a specific outcome. Senior living availability, pricing, and eligibility are determined by each individual community. We are not liable for decisions made by communities regarding residency or pricing, nor for any outcomes arising from placements made through our referral service.
